New Patients

We are currently accepting NHS patients.

If you live within our practice area (see image below) you are welcome to register with us.

You can register online using the NHS GP Registration service.

You can also download and print off a registration form (PRF1) and bring it in to the Practice.

In order to register you at the practice, we require you to complete the relevant registration form(s). These forms are available from reception and can be downloaded online (please see below).

Please return the completed form, WITH your NHS number if you have been registered with the NHS before. Forms will need to be handed in to reception. We will need to see photo ID along with a letter dated within the last 14 days. This will need to include your name and current address on it. The registration form will then be checked by a receptionist to make sure we have all the correct information to be able to register you.

For children under the age of 16, please ensure you bring a copy of your Child’s Immunisation Record (known as the “RED BOOK”)

Please note: We may not be able to accept this registration form without it.

Acceptable forms of Photo ID:

• Passport.
• Driving Licence.

Acceptable forms of Proof of address (must be dated in the last 4 weeks)

• Utility Bill.
• Bank Statement.
• Rent Agreement Letter.
• Solicitors Letter.

Note: We may also accept medical cards as a form of ID, these must have an up to date address on it.

Patients Under the age of 16:

Please print and complete the following (3) forms and return to reception along with a copy of your Child’s immunisation record:
